Ø W- 4 and I-9 Federal forms
Ø W- 4 NYS form
You must have 2 forms of ID for payroll: picture ID and Social
Security card.
You will be filling out a tutor application at the second Tutor Training
Workshop. In addition to the application you must have a recommendation from one of your professors. Please choose someone who can truly assess your strengths. Also, choose a professor from the area you will probably tutor most frequently. The recommendation form can be accessed on our website,
