Application

Ninth graders wishing to be admitted for the current academic year must submit completed application packets before October 30th. Eighth graders who wish to participate in the program as ninth graders the following academic year in September need to submit completed application packets by this March 30th. Incomplete applications will not be reviewed. Applications received after the due dates are processed if there are student enrollment vacancies.

1. Instructions for completing the application packet
2. Marist College Upward Bound Student Application (four pages)
3. Upward Bound Student Recommendation Form (Please print two copies as two separate recommendations are required)
4. Upward Bound Student Disciplinary Action Release Form
5. Transcript and School Records Request Form
6. Camp Health Examination Form for Children, Youth and Adults

Candidates must also submit copies of the following documents, which can be obtained through your school's guidance department (use form provided):

1. An official school transcript
2. NYS Grade 8* standardized test scores in English Language Arts (ELA) and Math
   (*Current eighth graders applying for admission may submit scores at a later date.)
3. Most recent report card for the current school year
4. An IEP report (when applicable)

The complete application packet will also include a copy of the student's Social Security card, income verification (when required), documentation of current immigration status for non-U.S. citizens, and an interview report by our staff.

Please be certain that all parts of the application are carefully filled out by a parent, or legal guardian. Incomplete applications will result in delays, and will not be reviewed until complete. Application materials must be sent to the Marist Upward Bound Admissions Committee, 3399 North Road, Poughkeepsie NY 12601-1387.
