Mandatory health forms for incoming students are due by June 30th for the fall semester and January 15 for the spring semester
A cover letter with directions is attached to each packet. Please make sure all forms are completed, signed and mailed (not faxed) to the appropriate address indicated on the cover letter. For Full-Time Undergraduate Freshmen and Transfers, make sure you have attached a copy of a physical (within one year), immunization record, copy (front/back) of insurance card and have completed and signed all forms. Please mail in forms together.
Upon receipt and review of health forms, a confirmation post card will be sent by Health Services to the home address of incoming full-time traditional and transfer undergraduate students. Please allow time to process.

Tuberculosis (TB) screening is mandatory for some international students, and recommended for all students. A TB screening form is included in the Mandatory Health Forms. If a student screens positive by country of origin or risk factors, a TB Assessment must be completed by a health care provider.
