Data Request Form

For all data requests, please visit our website first. If you are unable to find the data you are looking for, please fill out the request information below. Requests will generally be answered in one week depending on complexity. In many cases someone in our office will contact you to discuss your request in more detail.

In general, data will only be provided in the aggregate: All requests will adhere to guidelines set forth by the Family Educational Rights and Privacy Act (FERPA).

Please give us as much notice as possible when requesting data, for some requests take longer than others depending on the nature of the data.
