Special Events Guest Attestation

In accordance with Marist College's COVID-19 policies and procedures the College requires that all visitors complete the Self-Reporting Survey below before coming to campus.

Each member of your group will need to fill out this form individually.  If a member of the group is under 18, the parent or legal guardian of the person listed can fill out the form.
