Special Events Guest Attestation

In accordance with Marist College's COVID-19 policies and procedures the College requires that all visitors complete the Self-Reporting Survey below before coming to campus.

Each member of your group will need to fill out this form individually.  If a member of the group is under 18, the parent or legal guardian of the person listed can fill out the form.

Have you experienced any of the following symptoms of COVID-19 in the past 14 days?

Fever, chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea.

Have you tested positive for or been exposed to COVID-19 in the past 14 days?

For this event you must be fully vaccinated (at least two weeks past the completion of the vaccine protocol) or have had a negative COVID-19 approved 72 hour viral test prior to the date of this event. By clicking "yes" below you attest to the fact that you have one or the other of these completed.

By clicking SUBMIT, I consent to MARIST COLLEGE documenting this health screening information pursuant to the requirements of New York State's Mandatory Guidelines. I am attesting, under penalty of perjury, that I have answered the screening questions honestly. I understand and acknowledge that if I do not meet one of the Entry Requirements, I will be denied entry to the Event. I further understand that I must comply with Marist College's COVID-19 policies and procedures, including but not limited to, wearing a mask while indoors. Failure to comply with Marist College's COVID-19 policies and procedures, including but not limited to its mask policy, may limit or deny my attendance at Marist College events or gatherings. By attending an event or gathering at Marist College, I understand and assume the risks associated with COVID-19, thus removing liability from the College.
